Next steps for your Root Cause Diagnostic & Prescription Roadmap 

 

Thank you for securing your place in the Root Cause Diagnostic & Prescription Roadmap.

This process is designed to help you understand what’s been missing in your fertility care — and to give you a clear, personalised plan for what to do next.

To ensure we can use our time effectively, there are a few important steps to complete now.

Step 1: Book your diagnostic consultation

Please use the booking calendar link on this page to schedule your 90-minute Diagnostic Consultation.

  • Please click the button below and select your timezone after you select “Book Appointment”
  • Select "Telehealth" 
  • Choose your timezone
  • Click "next"
  • Select  'Initial Consultations' dropdown 
  • Select "Telehealth - Diagnostic Consultation"
  • Click "next" 
  • Then choose a suitable time for you.
  • Create a new client account
    ** If booking from any other country than Australia, please enter your email address when booking, as text message confirmations won't be sent to you.

Once your appointment is confirmed, you’ll receive a confirmation email with all the details and reminder emails.

Your booking isn’t complete until a time is selected.

Step 2: Complete the detailed intake form

After booking, you’ll be asked to complete a comprehensive intake form. Please send us your partner's name and email address so they can be sent one also. 

This form allows us to:

  • understand your full fertility and treatment history
  • review key lifestyle, health, and emotional factors
  • prepare properly for your consultation

Please complete this form (your partner as well) at least 48 hours before your appointment so your information can be reviewed in advance.

Step 3: Upload relevant test results and reports

You’ll be prompted to upload any relevant documents to the intake form, such as: 

  • blood test results
  • imaging reports
  • IVF or fertility clinic summaries
    • specialist letters

You don’t need to upload everything you’ve ever done — just what you have available. If something is missing, we’ll discuss it during your consultation.

Step 4: Watch the preparation video

Before your consultation, please watch the short preparation video provided.

  • how the diagnostic Consultation works
  • what this process can and can’t determine
  • how to approach the session, so it’s as useful as possible

What happens next

After your diagnostic consultation:

  • your personalised Diagnostic & Prescription Roadmap will be prepared
  • we’ll book in a Prescription Clarification Call session to walk through the plan in detail
  • you’ll have clear guidance on next steps, whether or not you choose to continue care

There is no obligation to proceed beyond this process.
